The Origins of CPCMG Cares
CPCMG Cares is the recently formed non-profit arm of Children’s Primary Care Medical Group (CPCMG) which was founded November 1, 1995. The original medical group was the coming together of six practices and 28 pediatricians in San Diego County. Over the past three decades we have grown to 180 clinicians (MD, DO, NP and PA), 30 offices in San Diego and Riverside Counties with a patient base of almost 300,000 kids. Along the way we have aligned with Rady Children’s Health Network as part of an integrated delivery system, become accredited via AAAHC as a network level medical care home and expanded our services to include mental health, weight management, lactation, video visits and so much more.
One thing has remained consistent over the past 30+ years, CPCMG’s dedication to serve all children. In addition to accepting most commercial health insurances, we are also the largest provider of care to kids with Medi-Cal in the state of California. Through the Tricare program we serve many thousands of military families. It is through this spirit of inclusivity and community dedication that CPCMG has become a leading medical group in California.
We have recognized that only a small portion of health care occurs in the office setting. Many of our patients and families’ have additional stresses affecting their health. Others have more than just medical needs. The general term for this is called Social Determinants of Health (SDOH). This can range from transportation concerns to food insecurity to literacy gaps among other areas. These are not the types of issues that a pediatrician can just write a prescription and solve the problem. This is why we formed CPCMG Cares. We will try to fill in gaps where there are short comings in traditional health insurance and medical visits.
As we begin this journey, we will try to focus on just one area of SDOH, childhood literacy. CPCMG has partnered over the years with the American Academy of Pediatrics to help with early intervention by giving children books at their well visits from birth through five years of age. With our present funding sources, we have been modestly successful. About one half of our offices have been able to accomplish this. Our hope will be, within the next few years to be able to fund free books to all children in all our offices.
Once we have accomplished our childhood literacy goals, we will branch out to tackle other SDOH concerns. We would appreciate any support to help take care of the “whole child” beyond their medical needs.

Social Determinants of Health and the Importance of Literacy
What are childhood social determinants of health?
Social determinants of health in children are the conditions in which they are born, grow, learn and live that shape their health and development. These include family income, safe housing, access to nutritious foods, quality education, neighborhood safety and access to healthcare. When these supports are strong, children are more likely to thrive; when they are lacking, children face higher risks of illness and developmental delays that can last into adulthood.
Why is early literacy access so important?
Early literacy is so important because it builds the foundation for language, learning and lifelong success. When children are exposed to early talking, reading and storytelling they develop the needed skills like vocabulary, comprehension and attention which support brain development and school readiness. It is also linked to better academic achievement, resilience and health outcomes in adulthood.
Our Board
Adam Breslow, MD, MBA, FAAP (CPCMG Cares CEO)
Dr. Breslow is the immediate past President & CEO Emeritus of Children’s Primary Care Medical Group. He has been a practicing pediatrician for forty years with his clinical time mostly in Chula Vista, CA since 1989. He has dedicated his career to serving all children independent of their health insurance, giving access to physical, mental and behavioral health concerns. He has been on staff at UCSD, Sharp, Scripps and Rady’s since 1986, serving in numerous leadership roles. From 1994-2000, Dr. Breslow served in the U.S. Naval Reserve working at Naval Medical Center San Diego and other facilities throughout the country. He believes in treating the “whole child” and is looking forward to CPCMG Cares assisting those in need of resources outside of what healthcare insurance can provide.
Dr. Breslow has been married for 33+ years to Patrice, a retired pediatric nurse. They have two children, Ted, 33, a San Diego County Sheriff’s Deputy and Jonah 31, a data scientist. They currently have one beautiful granddaughter, Juniper, born August 2025, to their daughter-in-law, Jacqui, a Pediatric Intensive Care Physician.
Milagros Jacobs-Kleisli, DO, MPH, FACOP (CPCMG Cares CFO)
As a multicultural, Peruvian descent immigrant, reading opportunities were sparse growing up. At a young age I recognized I wanted to make a difference in the world by giving life by enhancing health and reaching out to communities similar in my upbringing, this evolved into the pursuit of public health (University of Southern California) and pediatric medicine (Touro University Osteopathic Medical School and Phoenix Children’s Pediatric Residency Program). This combination bestowed upon me the fact that children are resilient and will learn in any environment when given the opportunity to read. Children’s Primary Care offers medical care in various underserved populations, to which I get to contribute to and share life among these communities. The opportunity to aid in the establishment of CPCMG Cares Foundation to offer books to kids who are underserved and underprivileged in our community is an honor, as I share in this raising, and know that one single book can ignite the pursuit of learning. As a mother of four, I have incorporated nightly reading since birth and I would like to enable parents to share this experience, regardless of financial setbacks, for it is the children that are our future.

I have had the opportunity to work with several foundations (Make-A-Wish Foundation, San Diego & Old Town Academy Parent Foundation) in various supporting leadership roles. I am honored to be involved with the creation and advancement of the CPCMG Cares Foundation. Starting with our commitment to advance childhood literacy, the foundation will serve as rocket fuel for CPCMG to give back to the communities we serve while supporting CPCMG’s mission of working together to restore, sustain and enhance the health and developmental potential of children.
